Word Press Infusionsoft Opt In form - New Form Fields?

I am linking my wordpress website with infusionsoft and want to create an Opt in form. I need form fields to be ‘first name’, ‘email’ and then ‘telephone number’.

However I only have the option for first name and email or first name + second name and email.

Is there any way I can add my own field?

With that plugin, unfortunately there is not.

My recommendation is to get Gravity Forms and the CRMperks plugin — gives you a ton of flexibility with the forms and also with the integration.
