Custom Order Fields Stopped Saving to Order Record

Hi, I’ve had custom order fields (custom fields on the order record, not the contact record) working properly for months, and they suddenly stopped working at different times in each of my 4 Max Classic apps over the last few weeks. Considering that no changes occurred on my end, and the staggered start dates of the issues, I concluded that it was a change on Keap’s side that was rolled out over time to all the apps.

The code in the middle HTML section of the Keap Order Forms:

The fields appear to work on the Order Forms (to the user) but the data in those fields isn’t saved to the Order record anymore. As I said, this was working fine (for months) until recently.

Any ideas on why this suddenly stopped saving the data in the custom order fields?
