We use the “Keap Official Opt-in Forms” to replace some other optin form widget that Keap no longer supports. The plugin produces an email Optin Form that adds the user to our Customerhub site. As functionality goes, it is adequate.
I, however, am not satisfied with its appearance. The form is roughly similar to our site’s design scheme. Attempts to modify it any further have not succeeded. In particular is the text color for the Optin Consent: the text color is white, on a light orange background; almost impossible to notice, much less read.
The plugin offers the Settings option “Custom CSS.” I have failed to find any resource regarding changes possible using the Custom CSS option. Keap has a page about the plugin, installing it, and a bit of other hand-waving usage.
I had a long and fruitless chat with support about the plugin. The result was that Keap is not responsible for the Keap plugin (“Out of scope”). So sorry.

Other than that, Keap has been notorious for launching things that aren’t their core offerings (aka WordPress plugins), and then not maintaining them (thus the 1.9 star rating for the plugin).
If the link above doesn’t help you out, I’d recommend grabbing Gravity Forms or WPForms and connecting them to Keap using an integration like Make or Zapier.
You’ll get much better look/feel and customization options, along with more stability.
