How can I log calls?

We call various people on a daily basis just as cold calls, and then have at least 15 appointments scheduled a day. We would like a more clear vision of how to track and log our calls. What happens in each appointment, or what was spoken about in a cold call.
Is there a place where we can see not only a report for this, but also a CLEAR and ORGANIZED way to note it all down?

While IS has notes that can be marked for calls, it doesn’t have any real “system” for managing calls and their results. Similarly, you can schedule appointments but it’s not really connecting all the dots per se. There are solutions out there that integrate with IS that make this much more seemless.
